1 : The BDL Prescription Sheet
Each case must be accompanied by a correctly completed BDL prescription sheet. The prescription sheet is the document you use to indicate the specifics of your order. We have designed this sheet for ease of use and clarity of communication. In order to fulfill your turnaround demands, we need you to prepare the prescription sheet as accurately and comprehensively as possible to avoid communication delays.
Follow these steps when completing a prescription sheet :
Field 1: Case Information
Confirm the accuracy of the Case Number and/or Patient Name, Laboratory Name, and Primary Contact Email. BDL will use this email address to communicate any questions we have concerning the case.
Field 2 : Services
Choose appropriate service(s) by marking the respective box(es). If you request additional services, please indicate the appropriate tooth number(s) to the side. Furthermore, multiple tooth number(s) should accompany multiple services where appropriate to alleviate communication errors.
Field 3 : Ceramic Shade Instructions
Confirm both the shade guide and shade you're requesting. Any "unorthodox" shades or special shading instructions should be clearly explained and diagrammed in this section.
Field 4-6 : Contacts, Pontic Design, and Buccal Margin
Indicate your preference(s) by circling the appropriate choice(s).
Field 7 : Tooth Numbers/Case Design
Use this section to clearly communicate your preferred restoration design. Mark any tooth number(s) and use the vacant space for any explanatory text.
Field 8 : Special Instructions
Use this section to accurately communicate any instructions we need to be aware of, and/or further express your desired preferences.
Field 9 : Enclosed Materials
Clearly mark the appropriate boxes, and use the vacant space for extra text.
Field 10 : If No Occlusal Clearance
Clearly mark the appropriate boxes, and use the vacant space for extra text.
